General Information:

Fees - $350 (U10 & U11)/$450 (U12 and up)

Games Played - Minimum of 3 games per team

Dates - July 23-26

Registration Deadline - June 30th, 2008

Location - Lakeside Sports Complex (Orem, UT)

ATTENTION ALL COACHES/TEAM MANAGERS:

Team check-in for the Second Annual StrikerFest Tournament will be on Tuesday, July 22, 2008 at Scoreboard Sports, 390 East University Parkway, Orem, UT from 4:00 pm to 8:00 pm.

As per UYSA requirements, the following paperwork will be required for each player.

- A copy of your current roster generated by the Affinity program.  (Available from your District Registrar.)

A copy of each player’s birth certificate, including an English translation for foreign language certificates.

- A copy of each player’s UYSA participant registration/medical waiver form.

- A laminated, valid player pass for each registered player.  (Exception:  Previous season’s cards will be accepted if current cards are not available.)

- A signed Participant Agreement, Acknowledgment of Risk Fom (available by clicking here)

- A laminated, valid coach’s card for each coach.

You will be allowed to have a maximum of 5 guest players on your roster.  You must have the following paperwork for each guest player.

- A copy of the player’s registration/medical waiver form.

- A copy of the player’s birth certificate, including an English translation for foreign language certificates.

- A signed Participant Agreement, Acknowledgment of Risk Fom (available by clicking here)

- A laminated, valid player pass.

- A completed Specialty Player action form.  (Available on the UYSA website.)

Any players or teams not presenting the required documents at check-in will not be allowed to play.  No exceptions can or will be made.  There will be absolutely no temporary player passes made, and no one may participate without a pass.
